Rates and Location
I charge $4.00 per hour per infant(ages 0-1). I charge $4.00 per hour for toddlers and other other children.
I can provide meals at $1.50 each. Or you are welcome to pack your own lunch.

Child Care Contract
Child Care Contract
This contract is entered into and between ________________, parents of _____________, hereinafter “Parents”, and Amber of Tooele Utah, hereinafter “Provider”, for the purpose of securing arrangements for child care for _____________________________________, hereinafter “Children.”
The Parents Agree:
1. To pay the provider the rate of $4.00 per hour per child for the child care services for their children.
2. To pay the provider on _______________ of every month.
3. To pay the provider $1.50 per lunch and $1.50 per dinner. Unless child brings own lunch and dinner.
4. To provide the following supplies for their child: diapers, wipes and an extra set of clothing. Also formula for infants as well as baby food.
5. To keep Child's' immunizations up to date and provide any emergency contact names and phone numbers.
6. To call the Provider as soon as possible if the child is going to be absent. If 24 hour notice is not given, the Parents are still required to pay for the day the child is absent.
7. To have backup child care arrangements made in the event of an emergency or illness of the provider.
The Provider Agrees:
1. To provide child care services for the above named child for the hours and days stated below.
2. To provide a safe environment for the child.
3. To provide appropriate activities and toys for the child.
4. To communicate with the Parent about the needs and achievements of the child.
5. To not be held liable for any accident that happens to your child while in my care. (Example: If your child decides to jump off of my couch and breaks arm, etc.)
Both Parent and Provider Agree:
1. That the hours and days of child care shall be:
Monday___________________ Tuesday___________________ Wednesday________________ Thursday___________________ Friday_______________
2. That the Provider and Parent will give at least 30 days notice of any planned vacations or temporary closing dates of the child care.
3. That a minimum of one (1) month notice will be given by either Parent or Provider to terminate this contract.
4. That child care will not be provided if either the child or the Provider shall be considered to be too ill to receive or provide care.
5. That Parents may visit or call any time during normal child care hours to discuss or check on their child.
6. Due to problems in the past with check not clearing – The Parents will have to pay all of my bank fees PLUS a $30.00 fee to me for any returned checks.
7. To have a 'trial' 2 week period starting____________ and ending on _____________ to make sure the Parents and the Provider have time to be satisfied with the arrangements. The Parent or the Provider may terminate this contract with no fees if done within the 2 week trial period.
